What are your co-pays?
*Co-insurance is the percentage you pay on an “allowed amount” permitted by the plan. Eg., if the allowed amount for a hospital stay is $1000, you would pay $200 (presuming you have already met your deductible).
Other covered services includes: bariatric surgery, chiropractic care, hearing aids (up to $2000/36 months per ear), infertility treatment, non-emergency care traveling outside of US, weight loss programs.
